2024 Youth Leadership Summit

I/We the undersigned parents, desiring that our child named above have the opportunity to visit Nashville, Tennessee, by participating in the 2023 TENNESSEE ELECTRIC COOPERATIVE ASSOCIATION, YOUTH LEADERSHIP SUMMIT sponsored by our local electric system and the Tennessee Electric Cooperative Association (TECA), do consent to our child taking such trip.

I/We further authorize and direct the Tennessee Electric Cooperative Association, through its Activity Director for said trip, to direct and supervise our child, and we further request that said Activity Director secure any medical or emergency services he may believe to be necessary or desirable for our child during such trip.

The undersigned do hereby forever release, waive and discharge and the Tennessee Electric Cooperative Association, and their respective officers, directors, members, employees, and volunteers from and against any and all liability, losses, compensation, damages, actions, causes of action, claims, expenses (including medical expenses) and costs suffered or incurred by or on behalf of the above-named child or ward while attending any Youth Leadership Summit activity including, but not limited to, any related travel, activities, lodging activities, dining activities or otherwise.

I/we hereby grant permission to Tennessee Electric Cooperative Association, to use photographs, likenesses, and/or videotape images of my/our son/daughter for publicity purposes related to this activity.

Participant Liability Waiver & Photo Release

Young Men's Christian Association of Middle Tennessee (Joe C. Davis YMCA Outdoor Center)

In consideration of myself and/or my minor child being allowed to participate in the activities and programs of the YMCA of Middle Tennessee (''YMCA") and to use its facilities (whether owned or leased), equipment and machinery, I do hereby waive, release and forever discharge the YMCA and its officers, agents, employees, volunteers, representatives, directors and all others from any and all responsibility or liability for injuries or damages resulting from my participation (or my minor child's participation) in such activities or programs or my (or my minor child's) use of such facilities, equipment or machinery, even if such damage or injury results from a negligent act or omission.
By signing below, I agree that the YMCA may photograph or videotape me and/or my minor child and the YMCA may use those photographs or video footage for its marketing purposes. I release the YMCA from any claim or liability related to that use, and waive all claims for myself, my heirs and assignees against the individual staff persons and the YMCA of Middle Tennessee.

By signing below I acknowledge that I understand and agree to the YMCA liability waiver and photo release stated above. This liability waiver and photo release is intended to cover my current and future participation at the Joe C. Davis Outdoor Center.

Having been selected by your electric cooperative, you are considered one of the top high school students in Tennessee. You will be serving as a representative of the Tennessee Electric Cooperative Association and its member cooperatives from the time you leave home until the time you return from Nashville. You have a responsibility to yourself, and to all the others, to conform willingly and cooperatively with the following reasonable expectations:

1. Be punctual. We have a very busy schedule and cannot hold buses or delay starting events if we are to see and do all the things we have planned.
2. You will be required to participate in all planned activities of the YLS. You will not be allowed to leave the group at any time.
3. You will be required to wear your identification badge at all times during the YLS.
4. You must be in your room at the hotel at the time designated by the Director or chaperones. You are not to leave your room after this hour except with special permission of the Director or chaperone. You will not be allowed to visit other rooms or to have visitors in your room after curfew. Room checks will be made each night. The hotel security guards will notify the Director if any of you disregard this rule. Curfew is 11 p.m.
5. You will share a room with one other YLS participant and will be expected to respect the rights of others. After curfew, you will be expected to stay in your room, be ready for bed and be reasonably quiet so that others may sleep.
6. You must remain at the hotel at all times except when you are participating in the planned YLS activities. You will not be allowed to leave the hotel to visit relatives or friends in the area. Nor are tour members excused to tour Nashville on their own.
7. Smoking, vaping, chewing tobacco, alcoholic beverages and/or any drugs are strictly prohibited on the YLS. If you take prescription medication, provide written notification on the medical release form of the medication name and prescribed dosage prior to departure.
8. Infraction of these rules governing the conduct of our YLS will be considered cause for terminating your participation and returning to the hotel for remainder of the Summit.

Clothing Requirements

Each of you will be representing your family, school, cooperative and state in your dress and conduct. We expect you to continue the high standards that have been set by the previous participants. Therefore, we ask that you be dressed neatly at all times.

1. Cutoffs, short shorts, running shorts, tank tops, midriff tops, ragged jeans, sagging jeans and other types of very casual attire may NEVER be worn during our YLS (regardless of how hot it may be). Nice walking shorts, jeans and tennis shoes may be worn during touring and the bus trip to Nashville.

Dress Up (Visits with Representatives)

Boys: Suits and ties, dress slacks/sport coats and ties
Girls: Dress or dressy pantsuits, heels or dress flats

Casual Dress (Touring)

Boys and girls: Walking shorts, neat jeans (no short shorts or flip flops allowed)

Cell Phones

Due to the large number of our group and to the tight schedule we will be observing, we request that you only receive phone calls in case of emergency. If that should occur, your family may call you or the YLS Director, Todd Blocker at 931-224-0555.

You will only be able to use your room telephones for calls to other rooms. You are not allowed to place long-distance calls from your room telephone. You are welcome to use your cell phones when we are not involved in group functions. We ask that all cell phones be turned OFF any time we are touring. Our schedule is full, and you may not be able to call at the exact time your parents are expecting.

In Case of Illness

If you become ill during the trip, be sure to tell the Tour Director or one of your chaperones immediately. If you are taking prescription medications, they should be listed on the medical release form you sent in prior to the trip.

Lost or Separated from the Group

In the unlikely event you should become separated from the tour group, call your co-op chaperone and let them know where you are.

Expenses

Tennessee’s electric cooperatives are pleased to sponsor this trip. They will pay for transportation, meals, lodging and group entertainment. You will only need money for special expenditures such as souvenirs, snacks, etc.
